Ontario County - Space Change/Relocation Request

  1. This form is used to request administrative review before there is a substantive change in the use of any space or relocation of one or more employees. This does not include those that occur as part of approved capital projects.

  3. Be thorough. List all employees to be relocated as well as their current and proposed locations. Describe any work you think may need to be done such as minor construction, repairs, painting, wiring, furniture moving, etc.

  5. Please note: This form does not replace formal work orders currently required for moving furniture, painting, etc.
